Insurance
At Pacific Eye Associates, we aim to make insurance simple and transparent so you can focus on your eye health. We accept most major insurance plans, including Medicare, Medi-Cal, and PPO plans. Because coverage can vary, we recommend calling our office at 415-923-3007 and to confirm your specific plan before your visit. Our team is happy to help you understand your benefits and what to expect.
Coverage often depends on your specific plan type:
- PPO plans — We accept most PPO plans. PPO plans typically do not require a referral.
- HMO plans — Most HMO plans require prior authorization from your primary care physician (PCP) before your visit.
- EPO plans — We generally do not accept EPO plans.
Medical Insurance Plans and Groups We Participate In
Authorization is commonly required for patients under these groups:
- All American Medical Group (AAMG)
- Alignment Health Plan
- Chinese Community Health Plan (CCHP) / Jade Health
- North East Medical Services (NEMS)
- Sutter Pacific Medical Foundation
- On Lok
We are not in-network with the following, including: Hills Physicians Medical Group (any plans), Kaiser Permanente, San Francisco Health Network (SFHN), Meritage Medical Network, Oscar, Care1st Health Plan, and all student health plans.
If you’re unsure whether we accept your plan, we strongly recommend checking with your insurance provider or calling our office before your appointment.
If your plan is not accepted, you may still be seen as a self-pay patient. Payment is due at the time of service, and we can provide an itemized receipt for possible reimbursement, depending on your plan.
Vision Insurance
Medical insurance covers the diagnosis and treatment of eye conditions, while vision insurance is typically used for routine eye exams, glasses, and contact lenses.
For optometry services, we accept:
- VSP Vision Care (any Choice and Signature plans)
- EyeMed Vision Care (Blue View)
If you have a different vision plan, you’re still welcome to be seen as a self-pay patient. As with medical insurance, we can provide an itemized receipt for you to submit to your provider for possible reimbursement.
